The Nail in the Coffin quest automatically begins after completing "Teeth in the Bag" and speaking with Innkeeper Mole. He'll direct you to the Kuttenberg grandmaster.

Facing the Grandmaster

Find Frantishek, the Gravedigger, northwest of Kuttenberg, near the church. He'll only accept your challenge at night (after 9 PM).
