问卷调查系统的设计思路
又要求助于CSDN的各位大侠，
我想在一个系统上增加一个反馈（或者说问卷调查）功能，但是不知道该如何实现
想请各位大侠提供一点思路
具体需求是这样的：
我用WordPress搭建了一个信息发布系统，现在想在这个系统中的某几个特定页面中增加一份问卷，
问卷有多个题目，两种类型：单选 + 问答
问卷为匿名，提交问卷后只有管理员可以看到最终统计结果
鄙人没有做过开发，各位多担待
