L'application démarre mais est tuée parce qu'elle le fait ne se lie pas au port spécifié. Cela ressort clairement du message du journal Heroku :

Pour résoudre ce problème, le serveur Web doit être lié au port spécifié par la variable d'environnement PORT. Sur Heroku, le serveur HTTP est publiquement disponible sur les ports HTTP et HTTPS par défaut via les passerelles Heroku.
Par conséquent, l'application doit se lier au port spécifié au lieu de :9000.
